On vs Upon

The correct preposition in this instance is ‘up’!

Neither form you’ve given is correct.
“The decision is up to you” could be correct, if the meaning intended is that the decision is your choice or responsibility.
This form is colloquial.
‘The decision is up to you’, or, ‘It is your decision’.
